Ovarian torsion is one of the gynecological causes of acute abdomen in females of almost all age groups. It refers to incomplete or complete rotation of the ovarian with or without the involvement of adnexa. As it may cause subsequent ischemia and loss of ovarian and/or tubal function, expeditious diagnosis and management are the keys to obviating the complications and preserving ovarian function. Diagnostic Laparoscopy with detorsion remains the management of choice. Oophorectomy needs to be avoided wherever feasible. Oophoropexy should be considered to prevent future recrudescence. In this review, the focus is laid on the diagnostic approach in these cases to expedite the management and a literature review on outcomes after the preservation of a necrotic ovary and oophoropexy was undertaken.
